Cancellation Policy
Paternity DNA Collection Services Policy Black Diamond Notary, LLC Overview This policy outlines the terms for scheduling, canceling, and refunding paternity DNA sample collection services. By booking, clients agree to these terms. Appointments & Collection Appointments must be scheduled in advance. All individuals tested must present valid photo ID. For minors, a parent or legal guardian must be present and provide written consent. Samples are collected via non-invasive cheek swabs. Legal testing follows strict chain-of-custody procedures. Sample Shipping Collected samples are shipped to Choice DNA Laboratory via FedEx within 24 hours of collection (excluding weekends/holidays). Black Diamond Notary, LLC ensures secure packaging and shipment. Once delivered, testing and results are managed by the lab. Clients will be notified of any delays or issues. Cancellation Policy • Standard Appointments: Cancel at least 24 hours in advance for a full refund or free rescheduling. Late cancellations may incur a fee. • No-Shows: Failure to attend without notice may result in a no-show fee and forfeiture of refund. • Legal Testing: Must cancel at least 48 hours in advance. Late cancellations or no-shows may forfeit the full service fee. Refund Policy Refunds are available only for cancellations that meet the notice requirements. No refunds are issued once sample collection has started. Refunds are processed within 7–10 business days. Non-refundable items include travel fees, expedited services, completed collections, and legal testing fees once initiated. Privacy & Data Protection Black Diamond Notary, LLC is committed to client confidentiality. Personal data is securely stored and used only for testing purposes. Samples are shipped directly to the lab; we do not retain or test them. Information is never shared with third parties unless required by law. Clients may request access or corrections to their data.
